During the COVID-19 crisis we are providing remote wellness and telehealth physical therapy appointments.
Provide your contact information and we will contact you to setup a phone or video conference. Please note that if you are seeking physical therapy treatment versus just wellness or fitness sessions, you need to be located in the state of California due to the licensure of our physical therapists in this clinic. Sessions for the purpose of wellness/fitness do not carry this restriction.
New Patient/Client Paperwork – All new physical therapy patients and wellness/fitness Pilates clients need to fill out and sign this packet before your first session.
Remote Wellness Services Policies & Procedures Form – Please read, sign, and return this form prior to your first online session if you would like to receive wellness services from a ProBalance practitioner remotely (PT or Pilates teacher).
Telehealth Policies Form – This form must be signed and returned prior to receiving your first physical therapy TREATMENT via Telehealth.
